Starting Date and General information


Kindly note: R150 once off Activation and R100 Delivery charge (IF APPLICABLE) will appear on your 1st invoice (Payable by EFT). We require a minimum of 48Hrs to provision your Sim Card after we have received payment. Please allow for this. Sim Cards can ONLY be dispatched once provisioning is completed and the Sim is "Active" - Although we do not foresee any delays, DATA123 cannot be held responsible should network failures cause a delay in the provisioning. Incorrect Delivery Addres data and/or Failed deliveries will require the applicant to negotiate directly with the Courier Company to arrange suitable date, time and place for delivery.

Kindly insert the TOTAL number of sims here. The initial sim activation is once-off R150 and the additional sims have a once-off activation of R150 each PLUS R100 per month each.

I am aware that I am responsible to monitor my usage (via the data counter on my router/modem or by requesting usage from and agree to pay out of Bundle rates should I exceed my CAP applied for. Telkom Promotional product s on SoftCap packages carrier an R35/GB out of bundle cost.

I have checked my network coverage and am happy to proceed with this application. I am aware that Telkom Sims that offer roaming to MTN 3G , may have this roaming facility removed at any stage without prior notice from Telkom.


I am aware that, when an APPLE device is used for creating a Hot-Spot, the Telkom sim will NOT allow this.
